Our policy
Opal & Oak Psychiatry and Wellness PLLC complies with applicable federal and North Carolina civil rights laws and does not discriminate, exclude people, or treat them differently on the basis of race, color, national origin, age, disability, or sex (including pregnancy, sexual orientation, and gender identity).
Free communication and language assistance
We provide, free of charge:
- Aids and services to people with disabilities to communicate effectively with us, such as qualified sign language interpreters and written information in accessible formats;
- Language assistance services to people whose primary language is not English, such as qualified interpreters and information written in other languages.

Filing a grievance
If you believe we have failed to provide these services or discriminated in another way, you may file a grievance with our Section 1557 Coordinator at Opal & Oak Psychiatry and Wellness PLLC, 500 Benson Rd, Suite #225, Garner, NC 27529; 252-381-3152; or through the secure patient portal. You can file in person, by mail, or by phone, and we will help you if you need assistance.
Filing a complaint with the U.S. government
